- Measured at Months 6 and 12: Behavioural self-efficacy
- Patient activation
- Health behaviours, including exercise, physical activity, and smoking
- Health service use
Secondary Outcome Measures:
- Measured at Months 6 and 12: Health-related quality of life
- Body mass index (BMI)

- Ages Eligible for Study: 18 years and older
- Genders Eligible for Study: Both
- Accepts Healthy Volunteers: No
Inclusion Criteria:
- Currently receiving treatment at Dekalb Community Service Board
- Chronic disease
Exclusion Criteria:
- History of more than two psychiatric or medical hospitalisations within the year prior to study entry
